Privacy notice

What this website collects, why it is used, and how you can manage your choices.

Information we collect

When you submit the Practice Assessment, we collect the details you enter: your name, practice name, email address, phone number, state, practice type, provider and location counts, testing volume and process, clinical areas of interest, existing laboratory status, staffing capacity, timeline, role in the decision, primary reason, biggest concern, and any comments you add.

We also record the date and time of submission, the consent version you accepted, the page that referred you, and any campaign parameters in the link you arrived through, so we can understand which outreach is useful.

We do not ask for patient information, and you should not submit it. This site is not a clinical system and does not provide medical advice.

02

How we use it

We use assessment information to review your practice profile, decide whether an in-house laboratory is a practical fit, respond to your request, and prepare for a practice assessment call.

We use aggregate, anonymous measurement to understand how the site is used — which pages are read, where visitors stop, and which calls to action are used — so the site can be improved.

03

Analytics and cookies

This site does not load third-party advertising or analytics scripts, and does not write advertising cookies.

If you allow analytics in the consent banner, the site records anonymous events (page views, button clicks, form step completion, and form submission confirmation) in our own application database. Events contain an anonymous per-tab session identifier, the page path, referring page, campaign parameters, and the action taken. They never contain your name, email, phone, practice name, or any answer you typed.

If you choose “Essential only”, no first-party conversion events are recorded. Your choice is stored in your browser's local storage, not in a cookie. Use “Analytics preferences” in the footer to change it at any time. Hosting providers may retain operational logs for security and service delivery.

04

When information is shared

Assessment submissions are stored in our managed database and reviewed by the people responsible for practice assessments.

We may connect a CRM or scheduling provider to receive assessment details so the practice can be contacted and a call arranged. When such a provider is connected, it receives only the assessment details needed for that purpose and is bound by its own contractual and privacy obligations.

We do not sell assessment information, and we do not share it for advertising.

05

Retention

Assessment records are retained while the practice relationship is active and for a reasonable period afterwards for reference, legal, and record-keeping purposes. Anonymous measurement events are retained in aggregate for site improvement.

You can ask us to delete an assessment submission that has not become a project.

06

Your choices

You can ask what assessment information we hold about you, request a correction, or request deletion by contacting us through the Practice Assessment form and referencing your submission reference number.

You can decline analytics without affecting your ability to use the site or submit an assessment.

07

Security

Assessment data is transmitted over HTTPS and stored in a managed database with access limited to the people who review assessments. No system is perfectly secure, so please avoid including sensitive details beyond what the assessment asks for.

08

Scope

This notice covers this website and the Practice Assessment. It is not a business associate agreement, a notice of privacy practices for a healthcare provider, or legal advice. Laboratory operations, once established, are governed by the practice's own policies and applicable law.
